How to Apply for Admission

Admission and application to Maxville Manor is controlled by Ontario Health at Home.

To be eligible, applicants must be at least 18 years old, have a valid Ontario Health card, and require 24-hour nursing care and supervision that cannot be met through community-based services at home

Step 1: Contact Ontario Health at Home
Call your local branch by dialing 310-2222 (no area code required) or call 1-833-515-1234 to reach the central line. A care coordinator will be assigned to guide you through the process, which will involve an in-person assessment of your health, functional abilities, and mental capacity to make decisions.
Step 2: Research and Tour Homes
You are allowed to choose up to five long-term care homes, which can be a mix of not-for-profit or for-profit facilities. You should tour the homes in person, if possible, to find the best fit for your needs, culture, and language. Step 3: Complete the Application Forms
Your care coordinator will help you fill out the required paperwork, including the Application for Determination of Eligibility. You will also need your primary care provider or doctor to complete a mandatory Health Assessment. Finally, you will submit a "Choice List" of your 1–5 preferred homes in order of preference.
Step 4: Manage Wait Times and Bed Offers
Waitlists vary significantly depending on the home, the level of care you need, and the type of accommodation (Basic, Semi-Private, or Private). When an appropriate bed becomes available at any of the homes on your list, you will receive a bed offer. You must accept or decline the offer within 24 hours
Step 5: Costs and Financial Subsidies
The government fully funds all nursing and personal care, but residents pay an accommodation fee for room and board. Rates are set by the Ministry of Long-Term Care and vary by room type. If you cannot afford the standard basic accommodation, you can apply for the Long-Term Care Rate Reduction Program once you are living in the home. 

 

What to Bring

You may bring personal items such as: special photos (we will hang them on the wall for you),a small comfortable chair, a favorite bedspread (which will add a personal touch).

Bring an adequate supply of comfortable wash and wear clothing.

On the day of admission, please bring ALL your medications that you take. It is important that you continue to receive them and it is important for the nursing staff to know what pills you are taking.

Emergency Contact / POA

It is important to have the name of one family member (or significant friend) whom we can contact for information or questions, or in case of an emergency. This person may be the Power of Attorney for Personal Care. A “Power of Attorney for Personal Care”, is signed by the Resident when he/she is competent to sign legal documents. It states who should make the decisions for the Resident when he/she are unable to make a decision. We encourage all new Residents to provide this document on admission.
